Osyka Elementary School Pre-Kindergarten to Sixth-Grade Early Registration for 2024-2025
Early Registration: May 6-17, 2024 from 9:00 a.m. to 2:00 p.m.
Requirements for Pre-K, Kindergarten, and New Students:
- Original Birth Certificate
- Updated 121 Immunization Record
- Social Security Card
- Two (2) Updated Proofs of Residency
Returning students need:
- Two (2) Updated Proofs of Residency
Please note: A Driver’s License cannot be used as proof of residency.
If you miss early registration, normal registration dates will be Monday, July 8, 2024-Wednesday, July 31, 2024
- Monday-Thursday 8:00 a.m. - 3:00 p.m.
- Friday 8:00 a.m.-11: 00 a.m.
When registering Pre-Kindergartners, Kindergartners, and New Students:
- Complete the 2023-2024 Enrollment Card (available at the school)
- Provide Original Birth Certificate, Updated 121 Immunization Form, Social Security Card
- Submit Two updated Proofs of Residency (Driver's License cannot be used)
When registering Returning Students:
- Complete the 2023-2024 Enrollment Card (available at the school)
- Provide Two updated Proofs of Residency (Driver's License cannot be used)
IMPORTANT MESSAGE:
Parents and Guardians, registration for the 2024-2025 school year is mandatory. Failure to register by July 31, 2024, will prevent your child from attending Osyka Elementary School when classes start in the fall. Registration will resume on August 6, 2024, at 8:30 a.m.
